Postmortem

On March 10, 2025, between 17:50 and 18:10 UTC, we experienced intermittent connectivity disruptions and origin forward errors due to a power supply unit (PSU) mishap at our fabric site in Washington DC (US-IAD), which impacted multiple customers.

The issue was caused by human error during data center maintenance, where a working PSU was mistakenly unseated instead of the faulty one. This confusion arose due to the similar appearance of two different PSU models and their distinct indicator lights. While the working PSU was re-seated at 17:52 UTC, the impact persisted until 18:10 UTC.

We take full responsibility for the error and are updating our procedures to prevent similar issues in the future.
